Creating a conditional feedback form

The basic form builder consists of 2 work modes: the ‘pool’ and the ‘flow’. The pool is the heart of the basic form and may be used exclusively, skipping the flow process altogether.

If you wish to create a conditional feedbacl form you will need to use the 'Flow', it will allows you to change the default order of questions, determined in the pool. Using the flow you will be able to create a form that changes dynamiclly according to the form filler's answers.

1- After completing to create the pool - press on the ‘Flow’ work mode button to enter the Flow.
2- Start creating the flow - in the form of a tree. Press on the ‘Add a Question’ icon to choose questions from the flow and add them to your flow one by one.
You can determine their order using the ‘move up’ and ‘move down’ icons in the toolbar.
3- You can also add groups of questions to your flow by pressing on the ‘add a group’ icon. You may also determine after what question the group will be displayed using the ‘move up’ and ‘move down’ icons.
(Notice that the group question may not be separated – they will appear as a batch).
4- You can turn the form into a conditional form by placing questions/groups under multiple question answer options.
For instance : if you have a multiple question ‘what is your gender?’ which has 2 option (male, female) you can select the ‘male option’ and add a question after it (for instance, ‘do you like to drive?’) . You can also choose the ‘female’ option and place a different question under it (for example, ‘do you like shopping?’).
This way you can add many conditions to your form.
5- If you wish to add an 'end form' sign after one or several questions in the flow, you can do so by selecting the question and pressing on the ‘end form’ icon ('stop'). (for example, if you wish to collect only men’s data, you can add an ‘end form’ sign after the ‘female’ option. This will make sure that when a woman is filling in the form it will reach the finish line once she fills in that she is a ‘female’. )

Basic Form - Display as single group

The 'display as single group' is a new feature of the basic online form builder. Once you create a pool of questions and publish the form it will automatically, by default, be displayed continuously: each question or group at a time. This new feature will allow you to display your form in one list of questions, on one page (with a scroll if needed).Please note that this feature may not be used when you want to create a conditional form, because the flow of the form is dynamic and depends upon the form fillers answers.

Creating a basic feedback form

In its last versions FormLogix has provided the ability to create a basic feedback form.
It now offers 2 kinds of form builders to choose from: The first is the WYSIWYG drag & drop form builder which is already familiar to us. The second is a new Basic Form Builder. This new tool is great for those who wish to create forms quickly and easily and are not so interested in customization or flexible design.

Following are steps to create a basic feedback form:
1- Enter the 'basic form builder' (create new form> basic)
2- Enter a form name in the 'form settings'.
3- Start creating the form's question pool: Press on the 'Add Question' icon and enter your first question. (for example: question text: name, question type: textbox).
4- Enter your second question (for example: question text:feedback, question type: listbox, answer options: satisfaction, complaint, question, other ).
5- Enter your third question (for example: question text:email address, question type: email)
6- save your form.
7- publish your form.

Adding an email validation to your email form

When someone fills in your email form you should always make sure that the email address he/she fills in is valid so you can easily get back to them. This means that the email address should be correct in its syntax and it must also exist (since there is no problem making up an email address or typing it incorrectly by mistake).
The email validation option may only be activated from within the "email element", so if you haven't got this element in your email form you will need to add it.

Take Note!
This option will not work in Safari/IE6/IE7 or in forms smaller than 220x220 pixels.


Steps to use the Email Validation option:

1) Enter your email form in the form creator.

2) Select your email element and go to the "element settings" on the right.


3) Turn on the "email validation" checkbox and the "mandatory" checkbox as well.
The reason for turning on the "mandatory" checkbox is that in order for the email validation you must be sure that the user filled it in.



4) Press on "apply".

5) Save the email form.

6) When a form filler will try to submit your email form a window will appear, asking him to validate his/her email address.
The user will then have to click on the link to send himself a validation email. Once this email arrives in the user's mailbox he/she will need to copy the key number in it to the validation textbox in the validation window - and only then will they be able to submit your email form.

Making your feedback form fields mandatory

When you create your feedback form (or any other email form for that matter) you should make sure the important data is filled in by making the relevant fields mandatory.
Usually the fields that are most important in a feedback form are the email field (so you can reply to the person), the name (so you can politely reply using the person's name) and the message/feedback itself (which is the essence of your feedback form).

Following are steps to make your fields mandatory:
1- Enter your feedback form in the form builder.
2- Select a field you want to make mandatory and go to its element settings on the right.
3- turn on the 'mandatory' checkbox.
4- press on 'apply'.
5- save the form.

Once the fields are saved as mandatory the form filler will not be able to submit the form before filling them in.

Adding private access fields to your feedback form

Sometimes you want to add comments/ratings/notes to your feedback form entry after it was submitted, without the form filler being aware of it. This can easily be done by using the 'private access' option.
Private access fields are fields which are placed in a container within the email form and are only visible and accessible to the form owner. Main usage is for adding remarks or ratings to entries after they are submitted. This option suits all kinds of forms, ranging from contact forms and feedback forms to order and support forms.

Following are steps for creating access fields:
1) Enter the web form you wish to add access fields to in the form builder .
2) Drag a container element.
3) Turn on its 'Private Access' checkbox in the element's settings.
4) Drag the elements of your choice in to the container.
5) Save the form.

Please notice that the access fields will not appear in published forms, only in the form owner's report.

Following are steps for entering the data in the existing access fields:
1) Enter the form's report page.
2) Double Click on a specific entry line to open the entry in edit mode (or choose 'Tools'>Edit).
3) Your access fields will now be showing and you will be able to enter data in them.
4) press on the form's submit button to save your data.

Clone your feedback form

Cloning a feedback form is a good way to create identical or almost identical feedback forms in the shortest and easiest way.
Once your original feedback form is done you can clone it and make the desired changes.
Steps to clone your feedback form:
1- enter your account
2- in the data center - press on the "clone" icon of the relevant form in the "form list".
3- A copy of your feedback form will now open in the form builder and you will need to give it a name in the 'form settings'.
4- make the desired changes (design, adding/removing element etc)
5- save the email form.
